Live Well Information and Support Service at St Joseph's Hospice

I was delighted to attend the official launch of the Live Well Information and Support Service at St Joseph's Hospice, and mark the beginning of Hospice Care Week.
The service, funded by the Big Lottery Fund through the Reaching Communities Programme and Macmillan Cancer Support, will provide personalised information and support, both within the hospice and out in the community to those who need it.
The drop in service has been developed for patients, family members of someone living with a life-limiting condition and carers, with a support desk located at St Joseph's Hospice in Hackney. The Live Well Team are available Monday to Friday, 10am to 4pm, to talk through concerns and point people in the right direction so that they can access the services they need; such as hospice services, bereavement support , complementary therapies and so much more.
The new Outreach Service will see the Live Well Team going out in the community to deliver presentations and workshops to community groups, host information stalls at local events and organisations and facilitate peer support groups. Workshops are delivered on topics such as hospice care, illness self-management, healthy eating, cancer and dementia awareness and coping with grief and loss.
